Referral Process: Questions and Answers


How do I make a referral for my patient?

At all of our clinics we take the standard referral forms. If the patient’s insurance requires pre-authorization please be sure that this is done prior to them scheduling an appointment with us. We welcome the opportunity to talk directly with providers regarding patients who are being referred into our clinic. Please call (206)987-3005 with questions.

Eating Disorders Program: To refer patients to the Department of Child and Adolescent Psychiatry and Behavioral Health's Eating Disorders Program please contact the intake office at (206)987-2760 for both inpatient and outpatient services. Following are documents describing the program:

I feel that my patient needs to be seen immediately, how do I facilitate their visit?

The best way to facilitate an emergent appointment is to call the Children’s Operator at 206-987-2131 and request the adolescent physician on call. There is always someone on call – 7 days a week. The on call physician will discuss the case with you and will help facilitate an appointment when needed.

What information do I need to send with the patient?

In some cases, we require prior treatment records prior to making an appointment. Please send all information relevant to the condition being assessed either through fax to our clinic (206) 987-3333 or with the patient to their appointment. In specific, please send copies of any relevant visits, laboratory work, x-rays and radiology reports. Other chart information that is helpful includes: growth charts and measurements for adolescents with suspected eating disorders, and school records and educational testing for adolescent school failure.

How will you be communicating with me?

We feel that it is very important to keep open communication with primary care physicians in the community. Please be sure to list a return address on your referral so that we may send you a copy from our visit. You should expect a paper copy of the initial assessment after the appointment. Please feel free to contact us directly at any time. The easiest way to contact us is to call the Children’s Paging Operator at (206)987-2000 and have the physician who saw your patient paged.
