Adapting and Implementing a Nurse care Management Model to Care for Rural Patients with Chronic Pain

Study Objective
To adapt and test a nurse care management model to provide comprehensive coordinated care for patients with chronic pain in rural communities.
Long Term Goal
To reduce geographic disparities in pain-related outcomes through dissemination of this comprehensive approach to chronic pain management.
Study Components
Care Coordination
Patients will be assessed for and referred to services for social needs, behavioral health, and specialty care. Nurse Care Managers will use a system called PainTracker to develop goals, track progress, and refine treatment plans with patients.
Cognitive Behavioral Therapy
Patients will attend 6-10 weekly/biweekly sessions with Nurse Care Managers to develop strategies to change thinking and behaviors around pain.
Tele-Enhance Fitness
As a part of their treatment, patients may be referred to an online, evidence-based exercise program called tele-EF. Patients who are referred to the program will attend exercise classes 3 times a week for 4 months.
