Application Process

It is the responsibility of the applicant to see that all the materials listed below are received on or before the application deadline of December 15. This includes:

  1. References
  2. Professional Title
  3. Bachelor degree
  4. transcripts
  5. certified english tanslations
  6. degree statements
  7. official gre and toefl score reports
  8. These items should be sent directly to the irtphi office except the official gre and toefl scores, which should be sent directly from educational testing service to the university of washington graduate admissions office.

REQUIREMENTS

1. Application for admission to the International Research & Training Program in Health Informatics

2. A statement describing professional background, specific goals for health informatics-related training, and health informatics career goals.

3. Current curriculum vitae including a list of publications.

4. Copies of health informatics-related publications (if applicable)

5. Three letters of recommendation from individuals who can attest to the applicant's academic and professional capabilities.

6. Application for Admission to the Graduate School. This form should be sent directly to the IRTPHI office along with the other application materials. The Graduate School application fee will be paid by the IRTPHI for applicants who are accepted by the program.

7. Two sets of official transcripts. Official transcripts from all institutions of higher learning attended (i.e., undergraduate, graduate, professional). Both sets of transcripts should be sent to the IRTPHI office along with the other application materials. If the transcripts are in a language other than English, a certified translation must be sent along with the transcripts. These must be original documents. The University of Washington Graduate Admissions Office will not accept photocopies.

8. A Degree Statement. A degree statement for each post-graduate degree earned. This document is issued by the institutional register to verify the date of graduation and the title of the degree awarded.

9. Graduate Record Examination (GRE) scores

If you have not taken the GRE test within the last five years, you must register to take this test in time for your score report to reach the University of Washington by December 30. You can obtain information on registering for a GRE test at the Educational Testing Services (ETS) GRE web site at http://www.gre.org. Enclosed is a list of Regional Registration Centers, and you can contact the center for your region for further information, or write to ETS directly at the address below. You may also be able to obtain information at the American Embassy in your home country or a local university.

The University of Washington Graduate Admissions Office requires an official GRE score report, that is, a report sent directly from ETS to the University of Washington. Please be sure to request that your score report be sent to the University of Washington Graduate Admissions Office (institutional code 4854; department code 5199).

10. Test of English as Foreign Language (TOEFL) scores.

    Your TOEFL score must be current (no older than two years) and must be received by the University of Washington by December 30. Information regarding the test locations and dates offered is available at the Educational Testing Services (ETS) web site at http://www.toefl.org, or you may write to the Educational Testing Service at the address below, or contact one of the Regional Registration Centers. You may also be able to obtain information at the American Embassy in your home country or a local university.

Other Requirements UPON ACCEPTANCE to the program

  1. Applicants must submit a letter from the appropriate official of the home institution, which guarantees that the scholar will have a permanent position upon the return to the institution.
  2. Degree program candidates who do not have a current passing TOEFL score report will be asked to provide documentation of enrollment in English instruction in the home country.
