Extension for Community Health Outcomes

Videoconference Consultations

Each Wednesday, 12:00pm — 1:30pm PST, Project ECHO/UW Pain conducts videoconferences for community clinicians in Washington, Wyoming, Alaska, Montana, and Idaho that include:

  1. Didactics on useful chronic pain topics for primary care providers.
  2. Live case presentations from community clinicians
  3. Interactive consultations for providers with a multidisciplinary panel of specialists.

To participate:

  1. Contact Kent Unruh at ktunruh@uw.edu to receive connection information, and
  2. Complete the Pain Medicine Case Consultation Request Form and fax to 206-598-4576.
    Remember to:
    • Fax in the form by 5pm on Tuesday evening to be presented at the Wednesday videoconference.
    • Complete the whole Pain Medicine Consultation Request Form, including the a multidimensional longitudinal measurement tool, in advance of your presentation to our panel to improve the quality of our evaluation of your patient. The data will be shared and analyzed by all participants under the direction of our pain treatment experts. Follow-up and review of your patient's progress will be scheduled with you as necessary, also using this measurement instrument to track your patient's progress. This tool is embedded in the Pain Medicine Case Consultation Request Form (pages 2, 3, and 4 of that form).

When we receive your case, we will call you back with a confidential ID Number assigned to your case. Live case consultations will only refer to the patient by that confidential ID Number.

In summary, if you are a clinical provider and would like to present a case for consultation, please complete the Case Consultation Request Form and fax it to 206-598-4576.
