All affiliate fellows and all full fellows who received stipends are responsible for completing one Learning Contract per year.
Learning Contract Process:
- Complete the Learning Contract form
- Meet with your mentors (First year trainees also need to meet with the Director)
- Make necessary edits
- Obtain signatures of mentors (include the Director if you met with him)
- Send the signed contract to the Program Coordinator
- Send electronic copy to Program Coordinator
Progress Report process:
- Complete the Progress Report form
- Obtain signature of mentors
- Send the signed contract to the Program Coordinator
- Send electronic copy to Program Coordinator
