Timesheets
As you know, we recently changed our time sheets to comply with UW requirements.  Many of you felt the new time sheets were unwieldy, so in response to your concerns we have revised them once again to make them more user friendly.  Below are 12 monthly timesheets, one for each month in 2005 with weekends and holidays highlighted.  Also below is a Request for Leave form.  Please save these on your computer. 

All forms must be completed in hard copy, signed and returned to Ekua in J405 by the 10th of each month.  Instructions for completing the forms are as follows:

The color distinction for each job class has been abandoned.  All submissions should be on white paper. In each box Classified Staff and Professional Level 1 will indicate regular hours worked, e.g. W8, as well as deviations from the regular schedule, including over time worked.  Professional Staff Level 2 will indicate only exceptions to their regular workday.  In other words, you will enter only time away from work for annual leave, personal holiday, sick days, etc, e.g.A8.

Lastly, please enter the number of hours worked each week in the "Total" column.  The number of hours worked in each instance will normally be 40 since all leave is actually paid leave once you have passed your probationary period.  The common exceptions are Overtime for Classified Staff and Professional Level 1 whose overtime hours should be pre-approved and then added to the 40-hour workweek total. Hours that fall into the category of Leave Without Pay (LWOP) should be deducted from the regular 40-hour work week total for all job classes.

The Request for Leave form has been consolidated to include the most-frequently taken categories of leave.  The form includes sick leave.  Sick Leave does not require prior approval, but should indicated on the form.
