Hourly Timesheets
Hourly Timesheets Hourly timesheets can be found online through the Employee Self Service feature. When you become an hourly employee in the Department of Biochemistry Gail will set up an electronic timesheet for you. You fill in your own hours every pay period. At the end of the pay period you will use the Submit button to submit your completed timesheet and then notify your supervisor. Your supervisor will then approve your timesheet electronically.

If your timesheet is not submitted by the 10th and the last day of each month there is no guarantee of you receiving your pay on time for the pay period.

Under rare circumstances a paper timesheet may be needed to record your hours. Gail can provide you with a paper timesheet as needed.

Leave Record Sheets for Classified and Professional Staff
These record sheets are due the 10th of every month. In each box Classified Staff and Professional Staff–Overtime Eligible will indicate regular hours worked, e.g. W8, as well as deviations from the regular schedule, including over time worked. Professional Staff–Overtime Exempt will indicate only exceptions to their regular workday, only entering time away from work such as annual leave, personal holiday, sick days and so forth.

Please enter the number of hours worked each week in the "Total" column. The number of hours worked in each instance will normally be 40 since all leave is actually paid leave once you have passed your probationary period. The common exceptions are overtime for Classified Staff and Professional Staff–Overtime Eligible whose overtime hours should be pre-approved and then added to the 40-hour workweek total. Hours that fall into the category of Leave Without Pay (LWOP) should be deducted from the regular 40-hour work week total for all job classes.

Request for Leave Form
These forms need to be filled out for most types of leave from work such as Sick, Annual, Personal Holiday, and Civil. Please fill out this request form prior to taking your leave.

Pre-Travel Forms
Prior to your work related trip the University of Washington requires you to fill out a Pre-Travel Form. By doing so you will have provided the University of Washington and the Department of Biochemistry with a general estimate of the cost of your trip You will have also indicted what budget source will pay for the trip and assured the Department that the controller of the budget has provided prior approval of the funds being used for your trip, as that person will need to sign the Pre-Travel Form.
