In order to apply for certification, you must complete and submit the Certification Application along with supplemental materials which can be found on Catalyst Tools. To gain acess to Catalyst Tools you must have a UW NetID and password. Once we start accepting applications, we will provide instructions on how to create a UW NetID. In the meantime, the instructions below will give you an idea of how the process will begin.
Once signed in, click on the link marked “Certification”
From there you will see instructions and a list of documents you will need to submit for the application. You will want to set aside at least two hours to complete the application.
You will be expected to provide:
-Personal Information (including addresses, education, and job experience)
-Information on Clinical Training, Experience, and Supervision
-Your Knowledge of Mindfulness
-Information on Courses or Workshops You have Attended
-Supervisory Roles You Have Held
-Teaching Roles You Have Held
Additionally, you will need to provide:
-2 Letters of Recommendation
-A Copy of Each Professional License You Hold
-Professional References
-A Letter from Your Mindfulness/Zen/Contemplative Teacher (if applicable)
-Certificate(s) Awarded of Courses and/or Workshops
-3 Tapes of Supervised Sessions
