CPE Application Click here to download the CPE application. Please submit your application as one document that includes responses to each item listed on the application. Use Word and one 12 point font. Prior CPE evaluations may be submitted as separate documents. These may be emailed as a PDF or sent via US mail. The application fee is $20. Checks should be made payable to "Harborview Medical Center." You may submit your entire application by mail or you may mail your signed application face page and application fee to the address below and e-mail the rest of your application as a MS Word file.
