Complete and submit the following form to request a user account.

Accounts are available upon request to investigators from the CHARGE cohorts and members CHARGE working groups. By requesting and accepting an account on the site, users agree to treat all information found within as confidential and follow the procedures outlined in the Charge Principles and the CHARGE Working Group Principles.

Requests are reviewed twice per month by the Research Steering Committee. Once your account is approved, your account will be usable at Userlogin.

User account

A confirmation message will be sent to your email address once you submit this request. The address will not be published. Please respond by clicking on the confirmation link provided by the email. Finally, your password will be emailed to you when your account is created.

Personal information

Your biography will be set as the initial content for your userpage. Make sure you are comfortable publishing such information.

Try to include any relevant credentials in your biography below.

Please include some information about yourself. We require at minimum:

  • your name,
  • study affiliation, and
  • a list of CHARGE working groups in which you participate.

New users should also list contact information for at least one CHARGE working group leader who can help to verify your CHARGE membership.

The following information is kept private and will only be used for this request. You may want to list contacts such a phone number to aid in identify confirmation.
