Adolescent Health and Transition Project
Core Function: Technical Assistance and Outreach Training
The Adolescent Health Transition Project (AHTP) provides information to help smooth the transition from pediatric to adult health care for youth with special health care needs. The project is working to improve adult providers awareness of and willingness and ability to care for young adults with special health care needs in the local community and in subspecialty areas.
The project works with the Department of Health to examine issues related to health care needs for youth and young adults and to improve the access and ease of transitioning into adult health care.
The objectives of this project also align with the goals of the Maternal Child Health Bureau: (1) Providers will have the knowledge and tools to provide care for adolescents transitioning to adult care, and (2) Families and youth will have the knowledge and tools to provide care for adolescents transitioning to adult care.
The AHTP has linked with the Medical Home Leadership Network to conduct a needs assessment of physician practices which includes pediatricians and a few family medicine providers. This assessment provides information on the ease/difficulty of adolescent health transition in a number of local communities in Washington State.
Tools and information will be formulated, examined and streamlined to support the critical elements in a successful healthcare transition. These resources will be designed for all participants in the transition process which includes adult health care providers, pediatric health care providers, and families and youth.
Efforts are also underway to identify and support current efforts to provide health care to the young adult population with special health care needs. Thus far, the few transition programs identified are primarily in subspecialty clinics such as PKU (phenykotohuria).
