A number of records are maintained by CHS. These records are usually electronic medical records and are maintained within our Occupational Health Manager system (OHM).
Employees, students, volunteers, and visiting faculty should be knowledgeable that their "personal" medical information is maintained by their primary care provider (which may be UW Medicine within the UW Medical Center or Harborview Medical Center). These records are protected under the federal HIPAA laws and are not obtainable by the Employee Health Center staff or the Health Sciences Immunization Program staff UNLESS a signed consent is given by the patient for the retrieval of "specific" information only.
At the same time the records documented and maintained within OHM are protected as:
- Employee Health Records - these records are prepared and maintained under the auspices of State (WISHA) and Federal (OSHA) law for the benefit of the employee. Where these laws do not conflict, these records are also maintained in compliance with HIPAA.
- Student Health Records - these records are prepared and maintained under the auspicies of FERPA, federal law governing educational records. Where these laws do not conflict, these records are also maintained in compliance with HIPAA.
Please note that unless you sign specific consent forms, information within your CHS records will not be available to your primary care provider and vice-versa. This is for your privacy protection.
If you are both an employee and a student, or a student and a volunteer, or an employee and a volunteer, etc, then your CHS record is available to all CHS staff and sites whether it is an Employee Health record or a Student Health record.
Your CHS records are available to you and you may obtain copies (possibly at a small copying charge) through the site that you normally visit.
