Identity Theft Prevention Program

UW Medicine has established an Identity Theft Prevention Program (ITPP) to help us detect, prevent, and mitigate any identity theft of our creditors with covered accounts. UW Medicine entities collect registration and billing information to create patient accounts and/or bill for the provision of healthcare services. Patient accounts are a specific subset of covered accounts.  The UW Medicine policies for implementing its Identity Theft Prevention Program are posted below.

UW Guidelines for Preventing, Detecting and Mitigating Identity Theft set forth the University level ITPP (UW ITTP) and apply to all University Covered Accounts.
