The accounting of disclosure does not include disclosures that have been excluded under state and federal law, including:<\/p>\n<ul>\n<li>Disclosures made to carry out treatment, payment or healthcare operations;<\/li>\n<li>Disclosures made to individuals about themselves;<\/li>\n<li>Disclosures made that were incidental to a use or disclosure otherwise permitted or required for treatment, payment or healthcare operations (for example, when a copy machine technician pulls a jammed piece of paper out of the machine that contains PHI);<\/li>\n<li>Disclosures made to carry out an individual\u2019s authorization;<\/li>\n<li>Disclosures made for a UW Medicine entity\u2019s facility directory;<\/li>\n<li>Disclosures made to persons involved in the individual\u2019s care;<\/li>\n<li>Disclosures made for purposes of notifying one responsible for the individual\u2019s care and for which the individual had an opportunity to agree or object;<\/li>\n<li>Disclosures made for national security or intelligence purposes;<\/li>\n<li>Disclosures made to correctional institutions or law enforcement in custodial situations; <em>and<\/em><\/li>\n<li>Disclosures of a limited data set made in accordance with a specific data use agreement.<\/li>\n<\/ul>\n<p>Patients may request an accounting of disclosures by emailing <a href=\"mailto:comply@uw.edu\">comply@uw.edu<\/a>.<\/p>\n<p>UW Medicine workforce members may document disclosures in the Disclosures Accounting online application using a single entry submission via web form or batch data entry by uploading an Excel spreadsheet.<\/p>\n<ul>\n<li><a href=\"https:\/\/urldefense.com\/v3\/__https:\/disclosureaccounting.uwmedicine.org\/__;!!K-Hz7m0Vt54!lhOrMgcPLUyiCht0RkWvB4T622C9BSovTdoZ98lZVAisDV-lLbzizTT-7swycNj-ysPjy7-hDg-5aA$\" target=\"_blank\" rel=\"noopener\">UW Medicine Disclosure Accounting Website\u00a0<\/a><br \/>\n[Requires UW Medicine AMC Account]<\/li>\n<li>Having trouble accessing the website?
