How do I get cage cards for my animal transfers?
When you fill out a UW Facility to UW Facility Animal Transfer Request your cage cards will automatically be printed out for you and you will not need to request new cage cards.

Do not fill out a Cage Card Request if you are doing a transfer of any sort. Cage Card Requests are not replacements for filling out the Transfer Request form. Please download and follow the instructions the form specifies.

Do I need to tell you how many mice I have in breeding?
No. Only IACUC needs to know the breeding numbers. Please keep track of those for them, and contact IACUC if you have any questions.

How far in advance can I turn in an order?
You can turn in an order as far in advance as you can. Ordering in advance helps make it possible to get the exact animals you want without having to compromise on numbers or age. Also it allows for any problems like budget or protocol balance to be easily dealt with ahead of time.

If I am transferring housing and protocol, do I need to fill out both forms?
No. You only need to fill out the housing transfer form and just specify the protocol you want the animals on.

Who do I contact to see if my animals have arrived?
Check the rooms you requested to have your animals in to see if they have arrived. If you cannot find them check with the facility supervisor, or your lab personnel to see if they have arrived. For those animals that aren't housed in a UW provided facility, sometimes they have already arrived at your lab and just were not informed.

If I am making an order for multiple Investigators, whose name do I put on the order form?
There may be multiple Investigators in a lab and sharing the same protocol but there is only one Principle Investigator whose name is on the Protocol Number. Whenever there is a joint order or multiple Investigators, you always put the name of the PI who owns the protocol number being used.

Can I fax in my animal order?
No you cannot. Animal orders are accepted by email only. If you fax Animal Purchasing an order you will be sent an email notification and the animals cannot be ordered until the order is emailed.

How soon should I request new cage cards?
If at all possible, please request cage cards before the cages need them, this way the cages are never without a barcoded cage card and you will not be charged Tech time for our visits if the cages are without barcoded cage cards.
We understand when some requests cannot be made in advance. In these cases make the requests as soon as the cages are there and check with your facility supervisor on when the cards arrive. The shortest amount of time the cages are without barcoded cage cards the better.

Do I need to request cage cards for new animal orders?
No. Cage cards are created with the order you place and are provided by your facility supervisors upon arrival of animals. The information on the bar coded cage cards is provided by the order forms.

Will we be notified when there is a confirmation on delivery?
Not automatically. If you fill out the information at the bottom of the form you will receive a confirmation by incampus mail. You will not receive a delivery confirmation by email. However if your order has complications, is backordered or wait listed, we will inform you by email or phone. It is very important that if you order animals that you check your email daily.

Do I pick up cage cards at the Barcode Census Office?

No. Cage cards can be picked up in your Facility Supervisor or Program Coordinator's office. Please contact your Facility Supervisor for this information.

For all Facilities managed under Vickie Sillers, cage cards are delivered to her. Please contact her on how to receive your cage cards.
