  Online Cage Card Request Form  

Do not request cage cards for animal transfers. Your cage cards are automatically printed for you after Transfer Approval has been given. Cage Card Requests are not a replacement for submitting an animal transfer. If you are changing rooms, facilities, budgets and/or protocols, you must fill out a Transfer Request. Please see the Animal Transfers page. Requests should be provided at least 48 hours BEFORE the cage cards are required.
You request cage cards when:

    1. Breeding Colonies need seperated cages.
    2. Weaning pups.
    3. Cage cards are missing.
    4. Barcode is damaged or unreadable.
    5. Card is damaged and is unreadable (i.e. Water damage)
    6. Splitting cages (injured or sick animal isolation or separation in any way).
    7. Any other need for requesting cage cards.

Fill out the online form. All fields in red are required fields and must be filled in for your order to be submitted. After you successfully submit your form, an automatic confirmation email will be sent to you with the information you filled in. You have a limit of 120 cage cards per request and must order them in groups of 30. Your cage cards will be ready for pickup from the facility supervisor when an approval e-mail is sent. To activate the new cards, the barcoded activation strip located below the perforation is to be dated with the activation date, removed, and placed in the drop box inside the room. You are not charged for the cage cards themselves, it is a free and always has been!

DCM would like to reduce the amount of cage card waste as possible.Please do not request more cards than you will feasibly use, more cards can always be requested.
