If your barcoded cage card is damaged, especially if the barcode portion is, or if your barcode cage card is missing. If you have the cage card itself please input the cage card number that is below the barcode in the last field below and we can just reprint your current card.

Cage Card Requests are not a replacement for submitting an animal transfer. If you are changing rooms, facilities, budgets and/or protocols, you must fill out a Transfer Request. Please see the Animal Transfers page. Do not request cage cards for animal transfers. Your cage cards are automatically printed for you after Transfer Approval has been given.
