Dr. McClelland is the Site Leader for the Mombasa HIV/STD Research Site and the PI for the Mombasa Female Sex Workers Cohort, which was established in 1993. His research focuses on HIV-1 prevention in both seropositive and seronegative women. He is the PI on two ongoing NIH funded studies. 1) Antiretroviral Medications and HIV-1 Infectivity in Women. This study evaluates the time course, magnitude, and durability of genital HIV-1 suppression (a marker for infectivity) among women initiating and continuing antiretroviral therapy. The study also seeks to identify risk factors for genital HIV-1 shedding among women receiving antiretrovirals, including adherence, resistance, CD4 count, viral load, STIs, and hormonal contraception. In addition, the study will evaluate changes in sexual risk behavior among women on antiretroviral therapy, using the incidence of STIs as a biomarker for high-risk unprotected sex. 2) Reducing Vaginal Infections in Women at Risk for HIV-1. Vaginal infections have been associated with a significantly increased risk for HIV-1 acquisition. Because these conditions are highly prevalent, they may account for a high population attributable risk of HIV-1 infection. This randomized, double-blind, placebo controlled trial is evaluating the efficacy of monthly periodic presumptive treatment with metronidazole plus fluconazole for reducing the incidence of vaginal infections in a cohort of 400 women. Data from this study will inform development of a phase III efficacy trial of vaginal health interventions for prevention of HIV-1 infection. Research location: Nairobi, Kenya.
