Policies and Agreement

 

Communication: You can reach Project DATA Consultation services by calling (206) 221-4482. When you leave a message include in the message the best time to call you back. We will return your call within 1-2 business days. If your child’s behavior is a danger to the child or to others and constitutes an emergency please call 911. We are unable to provide emergency consultation or counseling.

Appointments:Appointments can be arranged at either the Experimental Education Unit, home or school. Call as much in advance as you can if you need to cancel an appointment. If we need to cancel an appointment we will call at least 2 hours in advance.

Charges and Billing:Parents and/or agencies are responsible for all charges. We do have several consultants who are Board Certified Behavior Analysts or who hold other relevant licenses. We will work with you if your insurance company reimburses for individual behavioral intervention, however we do not do third party billing. Bills for all services, except social skills groups, will be mailed at the end of the month and families are responsible for payment in full.

Fees for social skills groups will be prepaid in blocks of 10 sessions. Families forfeit sessions if their child cannot attend for any reason. However, if a session is cancelled due to staff illness, the session will be made up. Invoice Receivables sends past due notices to customers at 60 and 90 days based on the date of the invoice. Customers receive a final notice at approximately 120 days. Late payments may result in services being discontinued. Let us know if you are interested. Administrative decisions regarding financial assistance will be made by the management team.

Service Agreement: Services can be discontinued by parent/agency at anytime. If you have any questions, concerns, or are unsatisfied please notify your consultant or Dr. Ilene Schwartz. Please notify us as soon as possible if you wish to discontinue services. There is limited financial assistance available for families. Let us know if you are interested. Administrative decisions regarding financial assistance will be made by the management team.

Fees:

Behavioral Consultant for Families      $100.00 per hour
Educational Consultant                         (See fees listed in above description )
Social Skills Groups $500 for a block of 10 sessions ($100.00 per hour for additional family sessions)

 

FOR SHORT-TERM FAMILY CONSULTATION, THE FOLLOWING ACTIVITIES WILL BE BILLED:

• 2 hours for writing the initial evaluation

• Face-to-face meeting time with parents, child, teachers, or other professional service providers

• Phone or email consultation with parents, teachers, or other professionals over 15 minutes

• If visuals or other materials are needed there is a one time material fee of $25

• Mileage reimbursement less than 40 miles round trip. For trips over 20 miles one-way, travel time at the stated hourly rate will be billed in lieu of mileage

 

FOR EDUCATIONAL CONSULTANT, THE FOLLOWING ACTIVITIES WILL BE BILLED:

• See fees listed in above description

 

FOR SOCIAL SKILLS GROUPS, THE FOLLOWING ACTIVITIES WILL BE BILLED:

• Prepaid block of 10 social skills sessions, which includes a one-hour session for families

• Additional one-hour individualized social skills training sessions or play date sessions.
