General Patient Information
If you call for an appointment for yourself,  our appointment coordinators must first contact your obstetrical care provider in order to arrange for transfer of all pertinent medical information.  Then you will  be contacted by phone with the appointment information. 

Prior to your visit, you should prepare yourself to answer several questions: 

  • These include a medical history with specific attention paid to your history of diabetes.
  • Information concerning complications affecting prior pregnancies is important to review.
In order to enhance the care you receive, please feel free to download and fill in the general medical/obstetrical history  and bring it with you to your appointment. This form should not take more than thirty (30) minutes of your time to fill out.  You may also fax the forms back to 206-598-4694, Attention: Ms. Becky Harney, DIPP coordinator.

If an ultrasound has been been scheduled for you, you do NOT need to have a full bladder.  In fact, most likely you will be asked to empty your bladder before starting the procedure.

All information is confidential and will not be discussed with others, including family members, without your prior consent. 

Please feel free to download the Blood glucose and insulin log sheet for either pump delivery or intermittent injection of insulin.
