Information required from your office (the patient cannot be seen before receipt of the following):
- medical records about this and prior pregnancies
- prenatal records, including all ultrasound reports to date
- laboratory results, including serum screen, and maternal blood type
- patient blood sugar records
- insurance authorization/referral form
- Information about the status of the pregnancy will be relayed to the referring physician via a consult letter dictated by one of the perinatologists.
- Providers can also access the information about their patient via the UWMC secure web site service known as U-Link. This is an exciting and efficient method of obtaining feedback on all of your referrals. All physicians are encouraged to apply for U-Link privileges.
