III. 1998/99 UWMC/HMC Formulary Changes

The purpose of the UWMC/HMC Formulary System is to ensure that only the most efficacious of similar therapeutic agents are readily available for use within the institution.

The formulary changes listed in the table below are those which have been approved by the UWMC/HMC Pharmacy and Therapeutics Committee since the last reprinting of the Drug Formulary. These changes cover P & T Committee actions dated
April 1998- March 1999. The most recent changes approved by the Committee are shown in teal. (Note: Additions to the Formulary become effective on the 1st day of the following month.)
