Vaccine and Immunobiologics Information

Disclaimer

The National Childhood Vaccine Injury Act (NCVIA) requires that all health care providers who administer vaccines containing diptheria, tetanus, pertussis, polio, measles, mumps, rubella, hepatitis B, haemophilus influenza type b, and varicella must:

  1. Provide a copy of the appropriate Vaccine Information Statement (VIS) to the vaccine recipient, their parent, or legal guardian prior to each dose; and
  2. Report certain adverse events following vaccination to the Vaccine Adverse Events Reporting System.

Additionally, the NCVIA requires that health care providers document in the patient's permanent medical record:

  1. the date printed on the VIS
  2. the date the VIS is given to the patient or legal guardian
  3. the date the vaccine was given
  4. the vaccine manufacturer and lot number
  5. the name and address of the health care provider administering the vaccine
