Did you know… you need a permit for every time you want to serve food at your RSO meetings?

Instead of filling out a new form every time you want to have food at your weekly meeting, we worked with UW’s Environmental Health and Safety office to give you an easier option!  You may now submit one form per quarter for any food you want to serve each week!

So, if you want to have Domino’s deliver every week, or have Jimmy John’s two weeks out of the quarter, there is now an easy way to make sure you have approval.

To be approved to serve food:

1. Fill out the “RSO Weekly Meeting Food Form” and the “TempFoodEvent”/”ATFS” (Application for Temporary Food Service) form.  Get the forms from Erin or download them from this page below.
2. Submit both forms to Erin or the ECC Front Desk
3. We will fax them to the Environmental Health and Safety office to be approved.

Notes:

• If you’re only serving food once a quarter, you’ll only have to submit the regular “ATFS” form.
• If your group is hosting an event outside your regular RSO meeting time, you will need to submit a new food permit for that specific event if you want to serve food.
• Please submit ALL food requests AT LEAST two weeks before you serve food.  All requests must be approved before food may be served.

1. RSO quarterly food form – The ECC’s RSO food request form. So you can submit a single form for an entire quarter’s worth of food!

2. tempfoodevent – The UW’s official food service request form.  Submit one of these along with your “Quarterly Food form”.
