Program Guidelines
These guidelines contain information about the program, requirements for conducting 10- or 30-hour courses, resources that are available to help you with your training, and how to document your training to receive student completion cards.

How do I get OSHA course completion cards for my students?
If you received your trainer card from the Pacific Northwest OSHA Education Center:
After conducting an outreach training course, submit documentation about the course to us by clicking on this link: Electronic Card Request Form. Or you download a pdf of the required form by clicking on the following links: Construction Outreach Training Program Report, the General Industry Outreach Training Program Report, the Disaster Site Worker Outreach Training Program Report, or the Maritime Outreach Report (please fax or mail Maritime Reports).


Submit a separate request for each course you teach. You also need to keep a copy of the documentation for your records. After receiving your documentation, it may take up to three weeks to process your request.

Subject: New 10-hour cards

  • 10-hr Construction and 10-hr General Industry cards have been redesigned by Federal OSHA. You will see the new OSHA logo on each card and a new serial number series. New cards will be sent from this point forward.
  • The new 10-hour cards have a different format, so you will have to update your templates if you use a printer to print out your cards. 30-hour cards will also be redesigned shortly so please check this website frequently for updates.

  • At this time, you cannot use the same template for 10 and 30-hour cards. The most up to date templates (MS Word format) are posted below. When the new 30-hour cards arrives, the template will be updated at that time. Please contact our office if you need assistance.
