How do I get OSHA course completion cards for my students?
If you received your trainer card from the Pacific Northwest OSHA Education Center:
After conducting an outreach training course, submit documentation about the course to us by clicking on this link: Electronic Card Request Form. Or you download a pdf of the required form by clicking on the following links: Construction Industry Outreach Training Program Report, the General Industry Outreach Training Program Report, or the Disaster Site Worker Outreach Training Program Report.
Submit a separate request for each course you teach. You also need to keep a copy of the documentation for your records. After receiving your documentation, it may take up to three weeks to process your request.
