Instructions for Downloading and Printing Application Forms

The T.A./Fellowship Application (Optional), the Letter of Recommendation, and the Application for Admission to Graduate School forms are now available for download in Portable Document Format (PDF).  To view and print forms in PDF format it is necessary to have Adobe Acrobat Reader Software.  If you do not have the Software, it can be obtained free of charge from Adobe (or go directly to the Acrobat download page by clicking on the following image).  Please follow their instructions for installation on your computer.

Get Adobe Acrobat

Once you have Adobe Acrobat Reader installed on your computer, click on the link to the form you would like to print.  When the form appears in Adobe Acrobat Reader, click on the printer button (the leftmost button on the toolbar at the top of the window).  Choose the number of copies you would like to print, then click on the OK button.  Repeat this procedure for each of the forms you would like to print.

If you have difficulties, or are unable to install the Adobe Acrobat Reader Software, visit the Adobe web site for technical support.

Letter of Recommendation Form

The Letter of Recommendation Form is available in PDF format from English Graduate Studies.  You will need one copy of the form for each of your three letters of recommendation.

Teaching Assistantship Application

The Teaching Assistantship Application form is available from English Graduate Studies. You will need to submit this form to be eligible for teaching assistantships and fellowships.  If you are unable to access documents in PDF format, there is also a Word format option.

Requesting Printed Forms via Mail

All materials and information included in the English Graduate Studies application packet are now available online.   Both the English Graduate Office and Graduate Admissions prefer applicants use the available online resources, but realize that this is not always possible.  If you are unable to use the downloadable/printable forms above or have difficulties accessing any of this information, you may contact English Graduate Studies by writing to the department, calling our office, or sending an e-mail request to englgrad@u.washington.edu.
