Admissions Information and Instructions for Madigan Residents
Dear
Madigan Residents:
When submitting your application for admission to the MPH program in Occupational
and Environmental Medicine, please follow the modified application instructions below.
Application
Deadline
To ensure priority consideration, Applicants should complete their
application file with the Department and the Graduate School by January 15th for
Summer quarter admission. Applications completed after January 15th may
be reviewed on a space-available basis, but consideration of
late applications
is not guaranteed.
Step 1:
Review Program Prerequisites
Before submitting an application, please review the admission
prerequisites for the MPH program. Please disregard the section
regarding additional requirements for applicants to the UW OEM residency
program. An applicant with exceptionally strong
qualifications, but who does not meet all of the prerequisites,
may still be considered for admission.
The DEOHS Admissions Committee and the Occupational and Environmental Medicine program faculty
will review and consider the complete file, which includes transcripts,
test scores, goal statements, and letters of recommendation.
The program may also ask you to schedule an interview after they
have reviewed your file.
Step 2:
Submit Application Materials to the UW Office of Graduate Admissions
The
following should be submitted to the University of Washington
Office of Graduate Admissions using the web-based application system:
- Application
for Admission to Graduate School: This application
should be completed
and submitted online.
When
completing the application, please apply for "summer
quarter" and
choose the "Environmental and Occupational Health
Sci - Occ and Env Medicine (MPH)" when asked to
choose a program from the drop-down menu.
After completing this form online, please print one copy to include
with the department materials listed below.
- Application Fee: A $45 application fee is required when submitting the online
Application for Admission to the Graduate School.
Please note:
If you are applying
to more than one department at the University, you will need to
submit a separate application and fee for each department.
Step 3:
Submit Application Materials to the Department of Environmental and Occupational Health Sciences
After submitting your application and fee to the UW Office of
Graduate Admissions, please submit the following materials to the Department
of Environmental and Occupational Health Sciences, Graduate Program Office,
Box 357234, Seattle, WA 98195-7234:
- Printed
and signed copy of your online Application for Admission
to the Graduate
School (submitted above).
- The Departments
Supplemental Application Form: This application can be submitted
online
or a pdf
version can be downloaded and submitted with the rest
of the department
materials.
Please indicate on the application
that you are applying for the "MPH in Occupational and Environmental
Medicine,
without UW OEM
Residency."
On the question that asks whether or not you will
require financial support to attend graduate school, please check "No"
and write "Madigan Resident" in the blank.
- One
set of official transcripts from all undergraduate and graduate
institutions you have attended (including community
colleges). Ask the
Registrar of each institution to mail to you one copy of
your official
transcripts in a sealed envelope. Do not open these
envelopes; the transcripts
will not be considered official if the Registrars
seal is broken
or tampered with. If your prior institution will not send
you official
transcripts, they may be submitted directly to the department.
If you are admitted and enroll, you will be required to send
an additional set of transcripts
to the Graduate School.
- A narrative
goal statement indicating your reasons for seeking admission to
this program and how this education fits into your
long-term professional
goals. The statement should be one to one and a half pages
in length.
- Four letters of reference from people familiar with your
academic goals and potential for achieving those goals, one of which must be written by either the department chair, the head of your residency program, or your internship director. There is no form for
letters of recommendation, but the letters should discuss your
potential for successfully completing the graduate program. These may
be sent directly to the department or can be mailed with your
other application materials if they are in sealed envelopes
that have been signed by the writer across the back envelope flap.
- Official Copy of Dean's Letter from Medical School
- Copy of USMLE or Medical Board Exam Scores
