Admissions Decisions, Enrolling in Department
& Obtaining A Visa
ADMISSIONS DECISIONS
- Epidemiology does not use rolling admissions.
- No Department decisions available until February; you will receive an e-mail by March 1, 2008.
- Postdoctoral, Global Health & MCH decisions usually made last.
- If you are admitted, you will receive a written letter informing you that the Department has recommended your admission to the UW Graduate School.
- The final admission decision is made by UW Graduate Admissions, which almost always follows the departmental recommendation, with the following exception: international students will not be admitted until the Financial Ability Form showing sufficient funds along along with all other required supporting documents by UW Graduate Admissions.
ACCEPTING AN ADMISSION OFFER
- Complete the Epidemiology Reply Form by April 15.
- When you receive your official offer packet from Registration, return the enclosed acceptance form to Registration with the required $100 USD tuition deposit by the deadline indicated in the packet.
- Do NOT send the deposit to Graduate Admissions or to Epidemiology.
- If you cannot find your official admission offer, send a check with the following infomration on the memo line:
- Your name, student number, tuition deposit.
- If you don't know your student number, email us to ask.

- The deposit will be credited toward tuition or other student fees, such as a bus pass.
- Students entering Summer Quarter do not have to pay the deposit.
- Students entering Autumn Quarter will not receive an enrollment packet or be able to register until UW has received this deposit.
- If your full tuition is paid by a fellowship, you may get the deposit back after your Autumn Quarter tuition and fees have been paid in full.
- The $100 is non-refundable if you do not register for the quarter to which you were admitted except for extenuating circumstances, such as illness, pregnancy, etc.
- If you are considering changing entry quarters or deferring, do NOT pay the deposit until you decide. See Changing Entry Quarter and Deferral Policy.
- Keep your offer packet of materials in a safe place for future reference.
OBTAINING VISA PAPERWORK
- No visa paperwork will be issued until Graduate Admissions receives your Financial Ability Form with the required signatures and documentation showing that you have adequate funding. See International Applicant Additional Requirements.
- Visa paperwork will NOT be issued until UW Registration receives your tuition deposit.
- UW will provide a DS-2019 form for a J-1 Exchange Visitor Visa only if you are funded by a national government.
- If you are funded by a university, you may petition epiapply@u.washington.edu for a J-1 visa. The UW International Services Office will decide whether to issue a DS-2019.
- A DS-2019 form will not be provided if you have a privately-funded fellowship.
- UW will not provide H-1B visa paperwork unless you hold a doctorate, even if you are offered a UW job.
- Other US universitiess have different visa rules; please do not ask for an exception to the UW rules as it will not be granted.
