Additional Materials Due by Final Deadline
Reference Letters, Transcripts, GRE and TOEFLE scores are note required to submit the on-line application, but they must be received by the final deadline for an application to be reviewed. See detailed information below:
Official Transcripts
- Required from all institutions where you earned a degree, studied 9 months or more, or studied courses relevant to health sciences and public health, including mathematics and quantitative social sciences.
- Not required from an institution where you only were enrolled for a few courses unrelated to the fields above.
- Order official transcripts in sealed envelopes.
- Do NOT open the envelopes. Forward them to Epidemiology at Contact Information.
- If the institution will not send official transcripts to you or time is short, request that they be sent directly to Epidemiology.

Official GRE Test Scores
- The GRE General Test is required from all applicants, except the following:
- Applicants to terminal MS or MPH who hold a US doctorate. (Applicants with foreign doctorates must take GRE).
- Applicants from the UW Medical Scientist Training Program (to Epi PhD) or UW Medical School (to Epi MPH), who should submit their MCAT scores instead.
- Order GRE scores to be sent directly from ETS to UW.
- UW Institution Code: 4854; Department code not needed. (If you used one, it does not matter.)
- Deadline for all test scores to be received by UW: January 1
- Computer test scores take about 4 weeks, not 10 days, to reach UW.
- Paper test scores require at least 6 weeks to reach UW.

- UW does not allow admission conditional upon achievement of acceptable scores.
Checking Application Completeness
- Check Deadlines for Application Submission and Additional Materials.
- Check Online If Your Application is Complete:
- Log into your on-line application at UW with your login name and password.
- Do not contact Epidemiology until AFTER you have checked on-line.
- You are responsible for reminding your recommenders and for checking transcript requests with your previous institutions.
- It takes at least seven days for transcripts mailed from the East Coast to reach UW.
- Allow Epidemiology at least one week to process mailed items before contacting us.
- To Expedite Paper Items: send paper items by express courier; priority mail is not reliable.
- To Expedite Test Scores:
- Obtain scores and percentiles from GRE or TOEFL by phone (fee required).
- Allow at least 10 business days after the test date before calling.
- Note: December 25 & January 1 are holidays in the US. December 24 & January 2 are holidays if December 25 or January 1 are on a Saturday or Sunday.
- Email epiapply@u.washington.edu your full name, test registration number, exam date, exam location, birth date, scores and percentiles.
- We can verify (but not obtain) your scores with ETS.
