- Recommended only for Maternal & Child Health MPH students and for postdoctoral clinician fellows who must begin in the summer
- There is only one course available for other MPH/MS students and it is not applicable to the MS Clinical Research Track or the PhD Program.

When You Can Apply - Deadlines by Applicant Category
INTERNATIONAL APPLICANT DEADLINES, regardless of degree sought:
Non-Degree Application

Complete on-line application submitted to the UW Graduate School, including:
- On-line
Application Form
- Statement of Purpose
- Curriculum Vitae or Resume
- Epidemiology On-line Application Form
- Names, Email and Contact Information for at least Three Recommenders
- Official Transcripts and Degree Statements received by UW Graduate Admissions Office.
- Optional Diversity Statement (if desired)
- Payment of the Application Fee Online. International applicants NOT eligible for fee waiver.
- MCH Supplemental Material, if applicable

Application must be totally complete, including:
- Official scores received by UW directly from ETS
- All references received electronically through the application system.
- Official Transcripts and Degree Statements received by the Epidemiology Department.

The School of Public Health will join SOPHAS for 2009 entry, so deadlines may be earlier.
