Direct UW Application Submission Instructions
I. General Directions
The following directions are for prospective students who are applying
directly to UW. If you are applying to multiple schools of public health,
you may wish to use SOPHAS, the national application system.
To apply for admission to the Epidemiology Department at the University of Washington applicants must send items to both the Depatment of Epidemiology and UW Graduate Admissions.

Admissions
Click on each linked item in advance of application to view additional
requirements. Be sure to read the Graduate Admissions instructions
within the online application in detail. References, test scores, and transcripts are due 2 weeks after
the online submission deadline.
- Graduate Admissions Applicant Profile (Requires entering educational history and personal information).
- Curriculum Vitae (CV) or Resume.
- Optional Disadvantage/Diversity Statement(truly optional)
- Official GRE Scores to institution code 4854 (department code not necessary).
- Statement of Purpose specific to UW Epidemiology.
- Epidemiology Supplemental Application.
- Recommenders’ complete contact information. (Email reference requests will be sent immediately.)
- 3-5 Letters of Recommendation.
- UW Graduate School Application Fee (currently $65) . (A waiver may be available based on need for a U.S. citizen or permanent resident. You must petition at the end of the application at least 2 weeks in advance of the deadline to have it evaluated.)

- Complete Set of Official Transcripts.
VI. Additional Requirements for International
Students or Holders of a Foreign Degree

- Send all of the following to UW Graduate Admissions:
- Official Transcripts & Degree Statements (including U.S. degrees) to Graduate Admissions.
- Copy of UW application form with original signature from international applicants.
- Financial Ability Form (specific form required AFTER application) and official financial documents from international applicants.
- Copy of official visa eligibility certificate, visa stamp, I-94, and any current visa forms, if applicable.
- English Language Requirements (to institution code 4854, department code not necessary) or IELTS scores from international applicants & permanent residents w/o Bachelor's in English.
- If applicable a letter from the institution certifying the bachelor's was entirely in English must be sent to Graduate Admissions with a copy to Epidemiology. See English Language for more information.
VII. Required for PhD Application without U.S. Master's (or higher)
- Direct PhD admission is very rare.
- Carefully review the Admission Criteria.
- If you meet ALL of the criteria, you must send a copy of your published paper to the Epidemiology Program Office or epiapply@u.washington.edu to be considered for direct PhD admission.
- Before the final deadline, email the above address to let us know whether you wish to be considered for the MS to PhD or the MPH to PhD if you are not accepted for direct PhD admission. If not, you will not be considered for masters admission if not admitted to the PhD directly.

Please DO NOT contact us about the completeness of your application before submitting it or for two weeks afterward. If you submit near the deadline, wait a week after the deadline to contact us to allow for processing.
