Direct UW Application Submission Instructions
I. General Directions
The following directions are for prospective students who are applying
directly to UW. If you are applying to multiple schools of public health,
you may wish to use SOPHAS, the national application system.
To apply for admission to the Epidemiology Department
at the University of Washington applicants must send items to both the
Depatment of Epidemiology and UW Graduate Admissions.

within the online application in detail. References, test scores, and transcripts are due 2 weeks after
the online submission deadline.

Students or Holders of a Foreign Degree
Read information at Graduate Admission Information and
Required Application Materials for International Applicants
- Send all of the following to UW Graduate Admissions:
- Official Transcripts & Degree Statements
(including U.S.
degrees) to Graduate Admissions.
- Copy of UW application form with original signature from
international applicants.
- Financial
Ability Form (specific form required AFTER application) and
official financial documents from international applicants.
- Copy of official visa eligibility certificate, visa stamp,
I-94, and any current visa forms, if applicable.
- English Language Requirements (to
institution code 4854, department code not necessary) or IELTS scores from international
applicants & permanent residents w/o Bachelor's in English.
- If applicable a letter from the institution certifying the bachelor's was entirely in English must be sent to Graduate Admissions with
a copy to Epidemiology. See English Language for more
information.
VII. Required for PhD Application without U.S.
Master's (or higher)
- Direct PhD admission is very rare.
- Carefully review the Admission Criteria.
- If you meet ALL of the criteria, you must send a copy of your
published paper to the
Epidemiology Program Office or epiapply@u.washington.edu to be considered for direct PhD
admission.
- Before the final deadline, email the above address to let us know
whether you wish to be considered for the MS to PhD or the MPH to PhD if
you are not accepted for direct PhD admission. If not, you will not be considered for masters admission if not admitted to the PhD
directly.
