Application Instructions
I. General Directions
The following directions are for prospective students who are using the SOPHAS system to apply to multiple programs, including the University of Washington.
If you are applying to UW only, please click here.
To apply for admission to the Epidemiology Department at the University of Washington through SOPHAS, applicants MUST complete online applications
for both SOPHAS and UW Graduate Admissions. Due to state law, only applicants who have submitted an on-line application to UW Graduate
Admissions
may be considered.

VI. Additional Requirements for International Students or Holders of a Foreign Degree

below to Graduate Admissions.
- Official Transcripts &
Degree Statements (including U.S. degrees) to Graduate Admissions (in addition to SOPHAS).
- Copy of UW application form with original signature from international applicants.
- Financial Ability Form (specific form
required AFTER application) and
financial documents from international applicants.
- Copy of visa eligibility certificate, visa stamp, I-94, and any current visa forms, if applicable.
- Official TOEFL (to institution code 4854,
department code not necessary), or IELTS scores from international applicants, permanent
residents, and US citizens w/o bachelors entirely in English. NO other
Engilsh tests accepted and no waivers availible. See country
exceptions.
- If your bachelor's was entirely in English from a non-exempt country, you must request your institution to send a letter
certifying that fact to Graduate Admissions with a copy to Epidemiology.
VII. Required for PhD Application without Masters Degree or Higher
- Carefully review the Admission Criteria.
- If you meet ALL of the criteria, you must send a copy of your published paper to epiapply@u.washington.edu to be considered for direct PhD admission.
- Direct PhD admission is rare. Meeting the requirements listed does not guarantee admission.
- Before the final deadline, email the above address to let us know whether you wish to be considered for the MS to PhD or the MPH to PhD
if you are not accepted for direct PhD admission. (If not, you will not be considered for master's admission if not admitted to the PhD
directly.)

you submit near the deadline, wait a week after the deadline to contact us to allow for processing.
