Unofficial Transcripts
- Upload PDF scans of all official transcripts online before submitting application by December 1.
- Include 1 grading scale information sheet for each institution attended.
Official Transcripts Required
- Send official English-language transcripts and diplomas in original sealed envelopes from all instituitions of higher education
attended to the Epidemiology Department.
- Transcripts or diplomas for completed degrees must show the degree was earned.
- If study abroad courses are listed on on a US transcript, you need not send a foreign transcript.
- The receipt deadline is December 15th; earlier is much preferred.
- Do not send medical residency transcripts.
- International students also follow the procedures below.
International Students and Foreign Degree Holders
- Official foreign and U.S. transcripts (if applicable) and degree statements (diplomas) from institutions of higher
education must be sent to Epidemiology.
- Transcripts are due no later than December 15. (Earlier is preferable to allow time for transcript evaluation.)
- Transcripts must be signed or sealed in ink by the proper school official.
- A degree statement or diploma must include the exact degree and the exact date it was awarded. (a diploma is not
required if the degree and sate are listed on the transcript.)
- If a transcript/degree statement is not in English, an English translation is also required.
- The translation must be signed or sealed in ink by the proper school or consular (or other government) official from the
appropriate country.
- A notarized copy of your own translation is NOT acceptable.
- A transcript service evaluation is NOT acceptable (but send a copy to Epidemiology).
- A non-English transcript may be translated by a UW foreign language faculty member for an hourly fee. See the UW Office Directory for applicable contact information.
- Transcripts must remain in the original UNOPENED sealed envelopes. If your institution does not provide documents in
sealed envelopes, then send the unsealed official transcripts.
- If your university provides only one official transcript and degree statement, you may provide a copy signed in ink
by a consular (or other government) official from the appropriate country. A copy notarized by a U.S. notary is NOT acceptable.
- South Asian Applicants: Official mark sheets are required.
- If you live in the Seattle area, you may visit Epidemiology to have a staff member copy your documents.
Ask the staff member to make a stamped copy of your documents.
- The hours are 9AM to 5PM M-F, but a prior appointment is required. Email epiapply@uw.edu to arrange.
- Parking on campus is very difficult and costs $15 in cash, although some of the fee will be returned if you park less than
2 hours. It's best to take a bus.
- Do NOT send high school or secondary school transcripts.

Another Official Transcript Required to Enroll in Graduate Degree Program
- To enroll in a graduate degree program at the UW, you must have an official transcript sent to the UW Graduate School after
admission.
- If you are admitted, you will be asked for a transcript from the institution specified in your online admission information that
granted you a bachelor's degree or higher.
- It must include proof that the degree was conferred so you may not be able to send this immediately.
- Do not request the transcript until you are admitted and the applicable degree has been conferred.
- It must be sent directly from the institution granting the degree to the UW Graduate School.
