UPDATE CONTACT INFORMATION

Update your contact information with UW as soon as you change your address and when you know your Seattle address.

• Visit www.MyUW.washington.edu to update your address and UW email in the registration system. (Epi cannot do this for you.)


• Aslo send your new contact information to epi@uw.edu


• If you will not arrive until shortly before your first quarter, you may use the Epidemiology address below. Do not send large packages to this address as there is no room in your mailslot.

• The Registration Office will mail your enrollment and UPass stickers to the mailing address listed with the UW about 2 weeks prior to each quarter


• If an incorrect address is on file, your stickers could be lost and it is hard to get them replaced. If you do not want to use your UPass sticker, return the sticker to have the charge removed.
