Graduation Procedure

  1. Be certain that you will have finished (including grading by the instructor) ALL coursework no later than the end of the semester (Fall, Spring, or Summer) after which you wish to graduate.

  2. Request a "Undergraduate and Professional Degree Application" from:

  4. If you are planning to graduate at the end of the fall semester, the deadline for submitting the Application for Degree to the Registrar's Office is October 1. If you plan to graduate at the end of the spring term, the Application for Degree must be filed with the Registrar's Office by March 1; and if graduation is planned at the end of summer session, then the Application for Degree must be filed by May 1. 

  5. When the "Undergraduate and Professional Degree Application" is returned to the University a "Preliminary To-Do List for Graduation" form will be generated; one copy sent to you and another to us. We list here any courses you must still complete before the degree can be awarded. 

  6. Prepare an update to your Portfolio and submit it to the program director by the end of the semester during which you wish to graduate. This summary document (2 or 3 pages) should include an honest assessment of what you have learned by participating in the program. You may want to review the original Student Handbook on Prior Learning Assessment: Instructions for Portfolio Development to get some ideas about how to document your learning. Forms you may review to reflect on this summary include:

    The Participant Summary Experience Self-Evaluation Forms  you completed after each of your nine pharmacotherapy weekend workshops, and

    The Experiential Learning Proposal Forms  and the Experiential Learning Assessment Forms  prepared for the practicum.

    This Portfolio Update is an outcome measurement tool. Think about how you have professionally evolved in the ExPharmD program. We would like to be able to demonstrate that the degree is earned and carries significant and demonstrable learning with it.

    There is no perfect format for this update. We want you to reflect on what you learned in the program (not the individual courses) and how that changed your practice (or not). We do NOT want a summary of what you did in each course. 

  7. When you have submitted your Portfolio Update, completed all necessary coursework, and grades have been assigned by the instructors, the Registrar's office will be notified and your diploma will be awarded.
