Fee Waiver Information and Forms

U-Pass:

The University has decied it CANNOT stop automatically billing you for a U-Pass.

To have this fee waived, simply return the unused U-Pass in the postage-paid return envelope as instructed, along with the U-Pass Charge Petition. Regrettably, international students have to provide their own postage.

Details: All matriculated students are sent a U-Pass each quarter they register. The fee is $37/quarter. The U-Pass comes with instructions and postage-paid reply envelope that allows for returns. If the U-Pass is returned immediately upon receipt, the $37 charge will be reversed. Unfortunately this had to be done each time a student registers for a course, assuming a U-Pass is not desired. Furthermore, international students will have to provide their own postage for returning their U-Pass as the postage-paid envelopes only work if mailed from within the U.S.

International Student Accident and Sickness Insurance Plan:

Health Insurance Overview

To have this fee waived download the Application for Insurance Waiver and submit as instructed.

Washington State Law (WAC 478-160-260) requires that all international students maintain adequate health insurance while attending the University of Washington. All regularly admitted, degree-seeking international students who are not attending the University as part of an exchange program are required to purchase the Accident and Sickness Insurance Plan designed for UW students and their dependents. The deadline to sign up for, change plans, or cancel insurance is Sunday, the 7th calendar day of the quarter.

Given that ExPharmD students often register for courses after the 7th calendar day of the quarter, they can have the Accident and Sickness Insurance Plan premium waived by completing and submitting the Application for Insurance Waiver. The waiver should be submitted immediately after registering for the quarter. While the waiver can be submitted on a quarter-by-quarter basis, students may prefer to check the annual box when submitting the waiver after the first course registration in autumn quarter of each year (i.e., July 15 - November 14).

International Student Orientation Fee:

To have this fee waived download the International Student Orientation Fee Waiver Petition and submit as instructed.

The UW provides an orientation program each quarter for international students who are new to the University. The orientation is not mandatory, but all international students are automatically billed the $65 orientation fee in association with their first course registration, regardless of whether or not they participate.

International students who do not plan to attend this orientation may complete and submit the International Student Orientation Fee Waiver Petition to avoid this $65 fee. The petition should be completed and submitted immediately after registering for the first ExPharmD course.

One Last Note:

In March 2004 we were informed that the system has been changed and any international student who is NOT in F-1 or J-1 visa status will NOT be charged for insurance or orientation. Hopefully all the problems of the past will disappear, but in the meantime, there may be a few stragglers who will show a charge on their accounts.

We will keep the above forms available until we are sure the system is set to avoid charging students.
