Individuals seeking disability benefits under Title II, or as children of certain workers (DAC), may apply in person. They also may apply on line or obtain the needed forms and mail them or bring to their local Social Security office.

When you apply you will need to have medical records regarding the disability, or the name, address and phone number of a doctor who is familiar with the disability.
