Faculty Evaluation of Student

At the conclusion of the Clerkship, all faculty and residents who provided in-depth supervision of your work will complete a Feedback & Evaluation Form. Please familiarize yourself with this form and ask your faculty for periodic feedback on how you are performing under this rubric.

Scores and comments from the feedback and evaluation forms will be the major determinant of your final grade. Click here to view the numeric criteria for final grades.

Other determining factors include:

  1. Adherence to the time and attendance policy: Failure to adhere to the policy may result in a fail grade.
  2. Completion of the required curriculum tasks: Failure to complete any part of the required curriculum, including timely entry of your experiences into your Assignment Tracker form may impact your grade.
  3. Examination performance: The exam is pass/fail. Your exam score will not effect your overall clerkship grade unless you fail the exam. The highest possible grade for students who fail the exam is a "Pass." The Family Medicine Clerkship does not offer re-examinations.

The Site Coordinator is responsible for summarizing your performance based on his or her observations and the scores and comments of other preceptors you worked with during your rotation, however the Family Medicine Clerkship Directors are solely responsible for assigning the final grades. The evaluation form is reviewed by the clerkship directors in Seattle for consistency between numeric scores and overall appropriateness of comments before assigning a grade.
