| Providence
ElderPlace is one of 40 PACE (Program of All-inclusive
Care for the Elderly) nationwide and the only such
program in the State of Washington. It is funded
by Medicare and Medicaid and is designed to deliver
high-quality comprehensive care in a cost - effective,
seamless manner to the frail nursing home-eligible
elderly who desire to live in the community. The
participants' plan of care is tailored to their
individual needs by an interdisciplinary team of
professionals including doctors, nurses, pharmacists,
social workers, physical, occupational and
recreational therapists and a host of auxiliary staff
at the Center and in the community.
Since its inception in 1995,
Providence ElderPlace has served as an educational
venue for geriatric medicine fellows both from the
University of Washington and the Geriatric Medicine
Track of the Swedish Family Practice Residency
Program. At various times, residents from the
Providence Family Practice Residency Program and nurse
practitioner students from the Seattle Pacific
University have had blocks of clinical rotations at
ElderPlace. As a Clinical Instructor at the
University of Washington School of Medicine,
Department of Medicine, Division of Gerontology and
Geriatric Medicine, I have had the pleasure to serve
both as a mentor for the fellows and residents and at
times, as a clinical tutor for the second year
students from the UW Medical School.

thrives, the staff and I will strive to provide
high-quality care to the community elderly and
learning opportunities for geriatric medicine fellows,
residents, medical and nurse practitioner
students.
