he Division of Gerontology and Geriatric Medicine at the Department of Medicine, University of Washington, has built a substantial long-term care nursing home practice at several area nursing homes as well as several outreach clinics in nearby buildings housing seniors. The practice features excellent continuity between patients and UW physicians, and it is an important component of our training program. The Long-Term Care Service (LTCS) physicians and nurse practitioners follow patients who are discharged from UW Medicine facilities into several area nursing homes as the attending providers-of-record. They deliver primary nursing home care, re-hospitalizing these patients at UW Medicine hospitals as necessary, and follow them in hospital and outreach clinics or return them to their original care providers after discharge from nursing homes. This is a coordinated effort with colleagues in the UW Departments of Psychiatry and Rehabilitation Medicine. Geriatrics fellows, residents, and medical students participate in LTCS patient care activities.

The LTCS can help with your patients needing skilled nursing home care or home care and will coordinate follow-up appointments and/or readmissions.  The service can perform anticoagulation (de novo, in process, or continuation of coumadin or LMWH); perform venous or arterial doppler examinations; administer IV antibiotics, TPN, and blood products; replace peripheral or central lines; provide complex wound care; have x-rays completed and sent to UW Medicine physicians for review; perform echocardiograms; and provide follow-up.  Patients discharged from Harborview Medical Center, UW Medical Center, Veterans Affairs Puget Sound Health Care System, Fred Hutchinson Cancer Research Center, or Children's Hospital and Regional Medical Center to the facilities listed below are admitted and followed by the LTCS.

These facilities are all approved UW Sites of Practice.  Arrangements are sometimes made for another UW Medicine primary care physician to follow patients in nursing homes, particularly at Bailey-Boushay House, and in those cases LTCS providers assist the primary care physician as needed.  Hospice care is provided in nursing homes as well as in patients' homes.  Other facilities may be added to the list as the service expands.  The LTCS also includes home care, working closely with affiliated home care agencies such as Visiting Nurse Services.
