Advance Directives ca<\/span>n provide clear guidance to health care providers and loved ones. It is important to let your physician and loved ones know your wishes for treatment should you ever be unable to express them and have the latest copy of your advance directive on file. Most health facilities assume you want all available medical treatment, including life-sustaining care, unless you direct otherwise.\u00a0<\/p>\n<p>There are two types of Advance Directives: (1) Health Care Directive (also known as Living Will) and (2) Durable Power of Attorney for Health Care. The <a href=\"https:\/\/wsma.org\/\" target=\"_blank\" rel=\"noreferrer noopener\" aria-label=\"Washington State Medical Association (WSMA) (opens in a new tab)\">Washington State Medical Association (WSMA)<\/a> offers advance directives\u2014both the health care directive and the durable power of attorney for health care\u2014to physicians and patients to help guide decision-making at the end of life.<\/p>\n<p><a href=\"http:\/\/depts.washington.edu\/hdcoe\/wp-content\/uploads\/2020\/04\/WSMA-Advance-Directives-July-2019.pdf\"><strong>Download Health Care Directive and Durable Power of Attorney for Health Care here<\/strong><\/a> (PDF &#8211; 4 pages)<\/p>\n\n\n<h4 class=\"wp-block-heading\">Once you&#8217;ve filled out your Advance Directives:<\/h4>\n\n\n\n<ul class=\"wp-block-list\"><li>Provide\ncopies of your Advance Directives to your healthcare agent and to your doctors\nso they can put&nbsp;them in your medical record<\/li><li>It\nis a good idea to review these documents when you have substantial health\nchanges because&nbsp;preferences can change. <\/li><li>You\ncan always&nbsp;complete&nbsp;new Advance Directives&nbsp;to reflect your\ncurrent wishes.
