Introduction

These checklists were developed by the Youth In Transition Project (1984-1987) at the University of Washington Division of Adolescent Medicine. They are based on a model developed by the Children's Rehabilitation Center at the University of Virginia. Use these checklists as an ongoing measurement of the independent skills your child achieves.
