Working Together for Successful Transition:
Washington State Adolescent Transition Resource Notebook
What is it?
This notebook is a resource on transition from adolescence to adulthood
for young adults with special health care needs and disabilities.
Audiences likely to find this information useful include families,
students, school personnel, community agency workers, health care
providers and other groups working with youth in Washington State.
How do I use it?
The notebook is organized by sections based on content such as post-secondary
education, community participation, and vocational rehabilitation.
Each section may be downloaded as needed for individualized use, as
resource handouts for groups, or to make an entire hard copy of the
notebook. The documents in the notebook are a sampling of transition
materials and resources generally found useful for our state's special
needs youth, their families and service providers. To maximize usefulness,
users are encouraged to add their own resource documents to the sections
and/or add additional sections to any hard copy of the notebook.
