The UW Measles Immunity Requirement Form can be downloaded from our website (see the FORMS tab) and faxed to 206-221-6628. Please read both sides of the form. Fill it out legibly and completely and be sure to INCLUDE YOUR STUDENT ID NUMBER on it.
