Your First Visit

Referral Required

Hall Health Center requires a referral to see a physical therapist. 

  • The referral must be faxed to the billing department at 206-221-0922 before the first appointment is scheduled. 
  • Please call the billing department at 206-616-7596 to make sure the referral has been processed.

New patients may register and make an appointment  by calling 206-685-1044. You may also do this in person at the Sports Medicine Clinic (ground floor) or at the  Patient Services Center on the main floor (1st floor) of our facility.

Check-in

For all appointments, you must first check in. For a physical therapy appointment, this should be done at the Patient Services Center on the main or 1st floor (directly opposite the main entrance).  After checking in, you may proceed to the Physical Therapy waiting area located on the ground floor.

NOTE: For your first visit, plan to arrive 15 minutes early to complete the registration process. Be prepared to show your insurance card and, if you are a student, your student ID card. Non-students who have no insurance are asked to provide a $125.00 deposit prior to their visits (this does not apply to non-students who do have insurance).
