First Visit

Registration and Appointments

  • Call 206-616-2495 or
  • Come in person to the Patient Services Center (PSC), which is located directly opposite the main entrance on the 1st floor.
  • All patients must check in at the PSC for every visit.

There is a charge for missed appointments, so please let us know in advance if you cannot be here at your arranged time.

Preparing for Your First Visit

First-time patients may register and make an appointment by calling or by coming to the PSC in person. When you come for your first visit, plan to arrive 15 minutes early to complete the registration process.

Please have ready:

  1. Insurance card
  2. Students need UW ID card
  3. Nonstudents who have no insurance coverage are asked to provide a $125.00 deposit prior to each visits.
