• Come in person to the Patient Services Center (PSC)
• All patients must check in at the PSC for every visit.

Hall Health PSC is located directly opposite the main entrance on the 1st floor.

Is this your first visit?

First-time patients may register and make an appointment by calling or by coming to the PSC in person. When you come for your first visit, plan to arrive 15 minutes early to complete the registration process. 

Please have ready:

• Insurance card
• Students need UW ID card
• Nonstudents who have no insurance coverage are asked to provide a $125.00 deposit prior to each visits.
