Online Mental Health Assessments
Our online mental health assessments are basic screening measures for depression, anxiety, AD/HD, eating concerns and substance use.
It's important to remember that these screening measures are not diagnostic instruments, but tools to help you pinpoint more specifically your concerns. In no way can these instruments be substituted for an evaluation made by a mental health professional.
- After taking the survey(s) that apply to you, you will have the opportunity to begin our web-based referral process and make an appointment with the Mental Health Clinic at Hall Health.
- Your answers to the screening will remain anonymous unless you choose to request a referral to the Mental Health Clinic at Hall Health.
- We will not re-direct you to an external site or ask unnecessary demographic questions.
- Access is limited to UW students, faculty, and staff, and requires use of a UWNetID to login.
