Visit the Patient Service Center

All patients must check in at the Patient Service Center for every visit

The Hall Health Patient Service Center is located opposite the main entrance on the first floor.

Cancellation, no show, and late fees

We urge you to keep your scheduled appointments. If we do not receive at least 24 hours (for a Monday appointment, call by Friday) advance notice of your cancellation or need to reschedule, you will be charged a $40 fee.

NOTE: Insurance does not pay for missed appointments or late cancellations.

Is this your first visit?

First-time patients may register and make an appointment by calling or by coming to the Patient Service Center in person. When you come for your first visit, plan to arrive 15 minutes early to complete the registration process.

Please have ready:

  • Picture ID
  • Insurance card
  • Students need a UW ID card
  • Non-students who have no insurance coverage are asked to provide a deposit prior to each visit.
