Review and Release of Medical Records

A written request (we have a form) from the patient is required to:

  1. Review your medical record.
  2. Release information from your medical record to yourself or another facility.
  3. Have records from another physician or health care facility sent to Hall Health Primary Care Center.

Copy Schedule: Release of Information Priorities and Time Frames

WE ARE UNABLE TO PROCESS YOUR RELEASE OF INFORMATION IMMEDIATELY. MEDICAL RECORDS REQUESTS FOR INFORMATION FROM PATIENT’S RECORDS WILL BE PROCESSED IN A TIMELY, CONSISTENT MANNER AS FOLLOWS:

Requests involving patient care:

  • Emergency care of patients: immediate processing
  • Urgent patient care: 1 – 2 working days,
  • Moderate patient care: 3 – 5 working days
  • Patient request to view own record: 5 – 10 working days
  • Subpoenas, court orders, depositions: within 14 days
  • All other requests: within 14 days

Records will be made available in a timely fashion not to exceed 15 working days. If records are not available at the time of request, patients will be informed of the status of their medical records and the medical record will be produced within 21 days from the receipt of the request. If the information requested does not exist or cannot be found the patient will be notified within 15 working days. (RCW) 70.02.080, 70.02.090

Cost Schedule for Copying Patient Health Information

Effective July 1, 2007 through June 30, 2009:

  • Personal copy $0.69 per page (excluding sales tax and postage)
  • Washington state sales tax applies (currently 8.9%, but subject to change by the state).
  • Postage varies according to the weight of the copies.
  • All other requests (i.e., legal, insurance) will be charged $0.96 cents per page for the first 30 pages, $0.73 cents per page for all other pages, plus $22.00 clerical fee for searching & handling (excluding sales tax and postage).
  • There is no charge for records being sent to another health care facility or physician.
  • Immunization Records: For questions regarding immunizations, please call 206-685-1018. The first copy will be provided at no charge.
