Prerequisites
- Statistics (one of the following): STAT 220, STAT 311, EDPSY 490, PSYCH 315, PSYCH 317/318, QMETH 201, or Q SCI 381 equivalent
- Anatomy and physiology with lab: BIOL 118 and BIOL 119 or equivalent
- Introductory management: MGMT 300 or equivalent
- Introduction to computer applications and information technology: CSE 100, INFO 100 or equivalent
- Medical terminology: Not offered at UW

Anatomy, Intro to Computers, Intro to Management, Medical Terminology, Statistics.Prerequisite completion is rated at the time of application. Full points are given for courses that have been completed and partial points are given for courses that are in-progress. All prerequisites must be complete by start of Autumn quarter.
Degree Applicants
Admission to the HIHIM major is competitive. The program only admits for summer quarter each year. For detailed information about the program and to download Evening Degree and HIHIM applications, please visit the Evening Degree Web Site at: http://www.evedegree.washington.edu
- Current UW students: You do not need to apply to the Evening Degree Program. However, you must submit an application to the HIHIM major by April 15 to start the following summer.
- Returning former UW students: If you have not been enrolled for more than one quarter (not including Summer Quarter), complete two applications: an Evening Degree, Returning Former Student Reenrollment Application and an application to the HIHIM major. To receive priority consideration, both applications should be submitted by April 15th.
- Transfer students: Apply for admission to both the Evening Degree Program (75 credits required) and to the HIHIM major (90 credits required). To receive priority consideration, both applications should be submitted by April 15th.
- International students: Apply to both the Evening Degree Program and to the HIHIM major by April 15 for the following summer.
Certificate Applicants
Admission to the HIHIM major is competitive. The program only admits for summer quarter each year.
A baccalaureate degree from an accredited college or university and a satisfactory grade-point average (minimum 2.5 is preferred but demonstrated levels of professional competency may be used to satisfy the grade-point requirement). There is more than one way to apply to the certificate program. You may request an application packet by calling UW Extension 206-685-8936, by emailing certificate@ese.washington.edu, or by printing the application from the web site listed below: www.extension.washington.edu/ext/certificates/hia/hia_hta.asp
The Certificate Program in Health Informatics and Health Information Management application must be accompanied by three copies of the following:
- Letter of Application
A typed letter of application, (250 word maximum), describing (a) your relevant experience, if any, (b) your interest in the profession, and (c) how you expect the certificate program will help you meet your professional goals. - Current Résumé
The résumé should outline your employment history and list academic institutions attended. List degrees awarded by date. - College Transcripts
Official college transcripts from the institution (one official and two copies), including the UW, which awarded your highest or most pertinent degree. Also include official transcripts from any college or university where you have taken prerequisite courses. - Prerequisite Status
A list documenting completion of, or plans to complete, program prerequisites. The list should note the college where each prerequisite was taken and the grade received. - TOEFL Test Score
International students must submit proof of a satisfactory TOEFL (Test of English as a Foreign Language) score for English proficiency and proof of financial resources. For more information on English proficiency guidelines, please see: www.extension.washington.edu/ext/certificates/hia/hia_hta.asp
Apply by April 15th each year to assure being reviewed. Late applications will be considered on a space-available through June 1st (contact the HIHIM Program). Applications are reviewed after April 15th.
Applicants will receive written notice of their acceptance, or non-acceptance after their admission materials are reviewed. Conditional acceptance may be offered to qualified candidates on a space-available basis.

All accepted applicants will be sent a registration form for first-term classes as soon as possible after their application is reviewed. Accepted applicants will be enrolled in the certificate program when their registration form and first-term payment has been received by UW Extension.
Registration for those accepted into the certificate program occurs on a first-come, first-served basis.
Accepted applicants should return their first-term registration form and payment as soon as possible.

Application Fee
